The Seriously Ill Patient : Part 1 - The Primary Survey

Recognising and Responding to the Seriously Ill Patient

Part 1 : Primary Survey

How we approach the management of the seriously ill patient in the first few minutes may significantly determine their chances of survival, risk for complications, and long-term disability. In seriously ill patients, we are frequently required to identify and act on life threatening problems such as airway obstruction, severe hypoxia, circulatory shock or hypoglycaemia before assistance arrives or have the opportunity to complete a thorough clinical assessment.

In this course we consider the process for the immediate assessment and intervention in the seriously ill adult and explore the emergency management of life threatening complications affecting the airway, breathing, circulation and neurological system. This is referred to as the Primary Survey.

Although later courses will explore the clinical features and treatment of many of the specific causes for serious illness it is critical to recognise that the first step in the seriously ill patient is to ensure a clear airway, correct hypoxia and shock and recognise and treat hypoglycaemia. These should be identified and corrected before focusing on the diagnosis and treatment of the underlying condition.

This course explores the following topics:

Part (1) : Immediate priorities in the assessment and treatment of the critically ill patient

Part (2) : Recognition and emergency treatment of airway obstruction

Part (3) : Assessment and intervention of the patient with severe respiratory distress

Part (4) : Recognition and intervention in the patient with circulatory shock

Part (5) : Assessment and intervention in the patient with altered conscious state
