There are few emergency presentations that are more scary than acute chest pain. It is certainly the cause for major clinical errors in emergency with patients every year sent home with one of the life threatening causes for chest pain and often not surviving to return to the ED. In other cases the patient who appears stable can suddenly deteriorate or even arrest requiring rapid mobilising of resources to deliver care to the patient. The take home message for the patient with acute chest pain - never underestimate the patient and be vigilant always. Treat acute chest pain with great respect as it will from time to time surprise even the most experienced and seasoned ED clinician.

In this module we explore the clinical approach the acute chest pain focusing our attention on searching out the clinical clues to life threatening diseases such as acute coronary syndrome or pulmonary embolism. This is critical on two accounts : Firstly we wish to avoid missing serious disease with a risk that the patient may be sent home. Secondly we know that in acute coronary syndrome the earlier we identify disease the earlier we can initiate interventions reduce morbidity and mortality. Time is critical in assessing the patient with chest pain and we need to approach assessment systematically to identify / exclude serious disease. In this module we take on this challenge and outline how to approach to these patients in a way that will allow early recognition of serious disease and trigger appropriate interventions that reduce morbidity and mortality.
