Acute abdominal pain is a common cause of serious illness. As the population ages, emergency care will be increasingly confronted with elderly patients presenting with acute abdominal pain. It is vital to have a sound approach to the assessment, investigation and management of these patients as presentations with serious pathology can be subtle and easily missed in this population. Important considerations when in assessing the elderly patient with abdominal pain include the high incidence of serious disease in this population and an appreciation that “atypical” presentations for serious and life threatening conditions are “typical” in this patient group.

In this module we examine the red flags for serious illness to the elderly patient with acute abdominal pain, identify the life threatening conditions that may present in this group and consider the role of investigations to assist identifying or ruling out serious disease. In most circumstances our management will be limited to fluids, analgesia +/- antibiotics with life saving surgical intervention required in a significant proportion of cases. Assessment the elderly patient can pose a major challenge due to the limited history, atypical presentation and the absence of major clues to serious disease. This module assist the clinician to develop a structured and informed approach to assessment that will reduce the risk of missing serious disease in this population.
