The patient with severe agitation can be extremely confronting to the clinician presenting not only major issues with respect to assessment and management but may also represent a risk to the safety of staff and patients. The differential diagnosis is broad and may be as simple as “a bottle of vodka” or as sinister as meningitis and a careful assessment of both their physical and mental health is essential. The immediate priority however is to reduce their risk of harm and identify serious and possibly life threatening medical causes for their presentation such as head injury, hypoglycaemia, infection, drug intoxication or withdrawal.
In this module we explore the immediate priorities in the severely agitated patient and introduce to you the "Mental Health Primary Survey" and then examine the procedure and pharmacological options for managing severe agitation. In the Extended care course we continue this discussion with an examination of Medical Clearance, Legal issues in relation to Detention and restraint and get up close with the Sedative agents that may be used to control agitation.
