Training Education Network (TEN)

Clinical Training for Medical Officers, Nurses, Nurse Practitioners and Paramedics

The TEN 360 Program provides a broad range of training and is suitable for both the novice learner and the experienced clinician. The program explores the diagnosis and critical thinking in the acutely ill or injured patient, delivers hands-on skill training for a wide variety of clinical procedures applicable to Primary Care, Emergency department and Inpatient settings and provides Certification Training in Advanced Life support, Newborn Advanced Life Support, Advanced Airway/Ventilation and Procedural Sedation.

The program has three components:

  • Emergency Workshops: These run over two days and explore the following areas of acute care: Cardiac Emergencies and ECG interpretation, Obstetric and Neonatal Emergencies, Major Trauma and Burns and Paediatric Emergencies. 

  • Competency Based Skills: One day workshops provide structured competency based training in Wound Management (Suturing), Orthopaedic Plaster (Splinting/Casting), Core and Advanced Clinical Skills. 

  • Certification Training: One day workshops deliver skills and simulation training allowing certification in the four key areas of Advanced Life Support (ALS2), Newborn Advanced Life Support (NALS), Advanced Airway/Ventilation and Procedural Sedation.

The program runs throughout the calendar year allowing learners to select the emergency workshops, clinical skills and certification training most relevant to their learning needs and CPD requirements. All courses are CPD certified with applications submitted for RACGP and ACRRM accreditation for the new triennium.

Emergency Workshops

Cardiac and ECG (2 day)

In this exciting workshop we explore a range of clinical cases that illustrate the presentation and emergency management of cardiac emergencies including myocardial ischaemia, pulmonary oedema, pulmonary embolism, arrhythmias and cardiac syncope. Core skills training during the workshop examines the set up, pressures and practical application of noninvasive ventilation for the management of pulmonary oedema and the use of synchronised cardioversion and pacing for management of cardiac arrhythmias. By the end of the two days you should be confident to recognise and initiate emergency management for the majority of cardiac emergencies you will encounter in your clinical practice. 

ECG interpretation is a major component of the workshop and identifies the ECG features of myocardial ischaema (including an introduction to recent  and less well recognised patterns) and a practical approach to the ECG diagnosis of arrhythmias. We cannot promise to make you an ECG expert but you will certainly be more skilled in recognising the clues to myocardial ischaemia and be able to narrow down the differential diagnosis of arrhythmia on the ECG.

Obstetrics and Neonatal Emergencies (2 day)

Let's face it - Obstetrics for the non-obstetrically trained is scary and the sick neonate - even more scary! In this workshop we will aim to tackle this head on by exploring a series of clinical cases on day 1 that illustrate the presentation and emergency management of obstetric emergencies including severe pre-eclampsia / eclampsia, placental abruption, preterm labour, preterm prelabour rupture of membranes, complications associated with labour and birthing, and post partum haemorhage. Core skills training on day 1 includes preparation and management of woman presenting in advanced labour to the ED and the management of birthing and complications such as shoulder dystocia.

On day 2 our focus is on the unwell neonate and we focus on the clues and red flags for serious illness and explore a series of clinical cases that illustrate the presentation, differential diagnosis and emergency management of airway obstruction, respiratory distress and circulatory shock in the neonate. Core skills training on day 2 includes basic airway management and ventilation in the neonate, set up of the neopuff ventilator and options for obtaining vascular access in the neonate. 

By the end of the two days you should be a little less scared when you encounter obstetric and neonatal patients and be more confident to assess, recognise the red flags for serious disease and initiate emergency management (while waiting for help to arrive).

Major Trauma and Burns (2 day)

In this workshop we take on the challenge of the patient presenting with major trauma and explore a range of clinical cases that will assist to illustrate many of the core principles for the recognition and management of major trauma from preparing to receive the patient, to the immediate assessment priorities and management of life threatening complications, to coordinating definitive care or retrieval. Managing the patient with serious / life threatening injuries is complex and requires a structured, highly coordinated approach to minimise the risk of missing serious injury, setting treatment priorities, determining which investigations will be most useful and ensuring continuous reassessment of the patient to monitor for deterioration.

On day 2 we explore major trauma in a range of patient populations including paediatrics and obstetrics and consider the assessment, management and potential complications associated with burn injury. Core skills training includes application of splints (eg pelvic binder, CT7) and torniquets, management of the c-spine (including log roll) and options for obtaining vascular access.

By the end of the two days you should be confident to prepare major trauma, identify the immediate priorities in assessment and management and follow a structured and coordinated approach to managing the seriously injured patient.

Critically Ill Child (2 day)

You would not be alone if you find approaching the seriously ill child a little daunting. The workshop will consider how to approach the many challenges for managing the critically ill child (especially in setting where paediatric resuscitation is uncommon) including subtle clues for recognising serious / life threatening disease, physiological and anatomical considerations, how to estimate weight, drug dosing, selection and administration intravenous fluids, preparing for paediatric resuscitation, inclusion of parents/carers, retrieval/transfer, assessment and management of pain in the child, managing non-accidental injury and considerations with regard to individual/staff well being. Core skills training during the workshop includes basic airway management and ventilation in the child, set up/use of high flow nasal prong oxygen, paediatric vascular access and simulations focusing on the assessment/resuscitation of the deteriorating child. 

On day 1 we focus on Paediatric Resuscitation and consider the clinical clues and red flags for serious illness in children and the structured approach to assessment and resuscitation in a child. The day will draw on series of clinical cases to explore immediate life threatening conditions that may present with airway obstruction, severe respiratory distress and circulatory shock.

On day 2 we extend our focus on serious illness and explore the assessment, differential diagnosis and management of the child with life threatening Neurological Presentations such as altered conscious state, status epilepticus, BRUE, diabetic coma and closed head injury. The day features a discussion of the new technology rapidly evolving in the management of type 1 diabetes and explore how this may contribute to the presentation and management of complications. We conclude day 2 with a discussion on the assessment and management of pain in children and approaches to providing sedation to facilitate procedures. 

By the end of the two days you should be confident to recognise and initiate emergency management for the majority of paediatric emergencies you will encounter in your clinical practice.

Competency Based Skills Training

Wound Management (Suturing)

For Medical Officers, Nurses, Nurse Practitioners, Paramedics

In this workshop we begin by considering the importance of wound assessment. While simple wounds are common in acute care it is always important to consider concomitant injury(s) or disease, risk factors that may complicate wound management and assess the wound for injuries to vital tissues such as tendons, nerves and joints. The aim is to identify those wounds requiring referral due to nerve, tendon vascular, joint injury or with extensive tissue loss from the simple wounds that can using be debrided and easily closed. The workshop focuses on the management of simple wounds (meaning wounds with no evidence of tendon or nerve injury or penetration of a joint). Major considerations that are covered include setup, preparation and equipment, local anaesthesia and haemostasis, decontamination and debridement, techniques for wound closure including suturing, wound glues and staples, antibiotics and tetanus, analgesia and post discharge advice and follow up. 

At the completion of this workshop you should be confident to assess a wound for serious injury and within your scope of practice be able to debride and manage the majority of the wounds you encounter in your clinical practice.

Credentialling for Advanced Nursing Practice

The program meets the SA Health training requirement for nurses wishing to become credentialled in advanced practice nursing practice. Upon completion of the workshop training, the nurse is required to be observed managing 5 patients requiring acute wound management (including at least three that require closure with sutures). The observer must be credentialled to suture patients in their clinical practice.

Core Clinical Skills (1 day)

For Medical Officers and Nurses

There are many procedural skills that are relatively straight forward to learn and where our scope of practice permits can be readily incorporated into our clinical work. This workshop provides hands on training for many of these core clinical skills and for nurses includes a standard competency based assessment.

Hands on clinical skills taught in the workshop include:

  • Male and Female Urinary Catheterisation

  • Nasogastric Tube Insertion

  • Intravenous Cannulation

  • Venepuncture and Arterial Blood Gas Sampling

  • Basic Eye : Examination and Procedures: VA, eyelid eversion, local anaesthesia, eye irrigation/morgan lens

  • External cardiac pacing and Synchronised cardioversion

  • High flow Nasal Prong Therapy 

  • Pleurocentesis and Paracentesis (Medical)

  • Competency Assessment: Male IDC, IV Cannulation (Nursing)

Orthopaedic Plastering (Splints / Casts)

For Medical Officers, Nurses, Nurse Practitioners, Paramedics

In this workshop we explore how to apply plaster splints and casts to manage a wide range of upper and lower limb orthopaedic injuries. A major focus is the hand on practising of a range of plaster techniques with the aim of being able to apply a splint that minimises the risk of discomfort to the patient (eg from local irritation or unnecessary restriction of movement distal and proximal to the injury), plaster failure due to poor technique and complications due to local pressure areas (beneath the plaster) and compartment syndrome. We will also consider the importance of selecting the correct splint for the injury. There are, for example a variety of splints that can be used for the wrist or hand injury including the dorsal backslab, volar splint, the ulna gutter and the thumb spica. The workshop covers the knowledge required to select the most appropriate splint for a particular injury and consider discharge advice and follow up for the patient.

Splints that will be taught and practised in the workshop include: Dorsal Backslab (upper limb), Volar splint, Ulna Gutter splint, Thumb spica, Scaphoid Splint and Dorsal backslab (lower limb). A range of other splints and casts may be discussed/demonstrated depending on the experience of the group.

Credentialling for Advanced Nursing Practice

The program meets the SA Health training requirement for nurses wishing to become credentialled in advanced practice nursing practice. Upon completion of the workshop training, the nurse is required to undertake 5 "observed" plaster splints on patients with acute limb injury and have these verified by a clinician skilled in splinting.

This is workshop is designed for nurses and provides hands on training in many of the advanced clinical skills required for nursing practice. The workshop incorporates competency based assessment.

Certification Training

Advanced Life Support (ALS2)

The Advanced Life Support (ALS2) workshop is designed for medical and nursing staff to be accredited/reaccredited in the skills required for Basic and Advanced Life Support with a focus on extending the knowledge and skills in Resuscitation/Stabilisation of the Critically Ill or Deteriorating patient. The workshop is delivered in a supportive learning environment and uses interactive online pre-learning to cover core knowledge and a one day workshop for procedural skills and simulation training.

The course is suited to health professionals who wish to extend their knowledge in management of the seriously ill or deteriorating patient in the emergency setting, met call, and rural and remote environment.

Topics covered in the program include:

  • Accreditation/Reaccreditation of Basic and Advanced Life Support

Newborn Advanced Life Support (NALS)

The Newborn Advanced Life Support (NALS) workshop is designed for medical and nursing staff to be accredited/reaccredited in the skills required for Newborn Advanced Life Support. The workshop is delivered in a supportive learning environment and uses interactive online pre-learning to cover core knowledge and a one day workshop for procedural skills and simulation training.

The course is suited to health professionals who wish to extend/renew their knowledge in resuscitation of the newborn in the labour ward, operating theatre, following (unplanned) birthing in the emergency department and in rural and remote environments. The training will assist to build the confidence and skills required to recognise and respond to a newborn requiring resuscitation.

Key skills that will covered in the workshop include:

  • NALS algorithm

Advanced Airway and Ventilation

The Advanced Airway and Ventilation workshop is suited to both novice and experienced learner and will be of value to medical officers, nurses, nurse practitioners and paramedics who wish to develop or consolidate skills in emergency airway management and provision of respiratory support  to the patient with respiratory failure.

The course will explore the features indicating a patient with a potentially problematic airway, pharmacological agents used in airway management, preparation and equipment  for rapid sequence intubation, basic and advanced airway techniques and front of neck access. Ventilation will consider the range of options for providing respiratory support and provide training in high flow nasal prong oxygen, noninvasive ventilation and ventilation in the intubated patient. The workshop is delivered in a supportive learning environment and uses interactive online pre-learning to cover core knowledge and a one day workshop for procedural skills and simulation training.

Topics covered in the program include:

  • Basic Airway management, Bag and mask ventilation

  • Advanced Airway management: Endotracheal Intubation, Front of neck access

Procedural Sedation

The Procedural Sedation workshop is suited to both novice and experienced learner and will be of value to medical officers, nurses, nurse practitioners and paramedics who wish to develop or consolidate skills in procedural sedation.

The course will explores indications, risk assessment, preparation of the patient, equipment, team for procedural sedation, common pharmacological agents and their routes of administration in procedural sedation, monitoring and recovery, recognition and management of complications, discharge procedure including discharge advice.
